Na pr\u00e1tica, mesmo com \u00f3culos atualizados, a crian\u00e7a n\u00e3o v\u00ea bem de um olho devido a uma falha no desenvolvimento visual normal durante a inf\u00e2ncia. Vejamos ambliopia e alguns tratamentos e alternativas ao penso ocular.<\/p>\n<p>As causas mais comuns incluem o <a href=\"https:\/\/www.paulofreitascosta.pt\/pt\/estrabismo\/\">estrabismo<\/a> (desvio ocular constante que leva o c\u00e9rebro a suprimir a vis\u00e3o de um dos olhos), a anisometropia (diferen\u00e7a acentuada de grau ou gradua\u00e7\u00e3o entre os olhos, fazendo um olho focar pior) e a priva\u00e7\u00e3o visual em um olho (por exemplo, <a href=\"https:\/\/www.paulofreitascosta.pt\/pt\/catarata-congenita\/\">catarata cong\u00e9nita<\/a>, ptose palpebral ou outra opacidade que obstrua a vis\u00e3o). Qualquer fator que interfira na vis\u00e3o de um olho nos primeiros anos (per\u00edodo cr\u00edtico do desenvolvimento visual at\u00e9 aos 6-7 anos) pode desencadear uma ambliopia.<\/p>\n<p>A ambliopia \u00e9 relativamente comum, afetando cerca de 2\u20133% das crian\u00e7as, sendo uma das principais causas de perda visual evit\u00e1vel na inf\u00e2ncia.<\/p>\n<p>&nbsp;<\/p>\n<p><b>Import\u00e2ncia do diagn\u00f3stico precoce:<\/b> \u00c9 fundamental detetar a ambliopia o mais cedo poss\u00edvel, idealmente em idade pr\u00e9-escolar, j\u00e1 que o tratamento \u00e9 mais eficaz enquanto o sistema visual da crian\u00e7a est\u00e1 em desenvolvimento. Muitas vezes, a crian\u00e7a e os pais n\u00e3o notam o problema (j\u00e1 que a vis\u00e3o do outro olho \u00e9 normal) e a dete\u00e7\u00e3o ocorre apenas em rastreios no pediatra ou na escola.<\/p>\n<p>Identificar a ambliopia aos 3-4 anos de idade permite iniciar atempadamente a terapia, maximizando a recupera\u00e7\u00e3o da vis\u00e3o. Os Rastreios visuais nesta faixa et\u00e1ria s\u00e3o altamente recomendados, dado que, sem tratamento precoce, a perda de vis\u00e3o no olho afetado pode tornar-se permanente.<\/p>\n<p>&nbsp;<\/p>\n<h3><b>Tratamento Convencional: \u00d3culos e Penso Ocular (Oclus\u00e3o)<\/b><\/h3>\n<p>O tratamento tradicional da ambliopia come\u00e7a SEMPRE pela corre\u00e7\u00e3o \u00f3ptica adequada \u2013 ou seja, usar \u00f3culos para corrigir quaisquer <a href=\"https:\/\/www.paulofreitascosta.pt\/pt\/erros-refrativos\/\">erros refrativos<\/a> (como <a href=\"https:\/\/www.paulofreitascosta.pt\/pt\/erros-refrativos-na-infancia-miopia-astigmatismo-e-hipermetropia-porto-canal\/\">miopia<\/a>, hipermetropia ou astigmatismo) e dar a cada olho a melhor imagem poss\u00edvel. Em seguida, o m\u00e9todo cl\u00e1ssico \u00e9 a a oclus\u00e3o do olho bom com um penso ocular, for\u00e7ando o uso do olho ambl\u00edope. Este m\u00e9todo monocular \u201cobriga\u201d o c\u00e9rebro a prestar aten\u00e7\u00e3o ao olho fraco e, de facto, na maioria das vezes resulta em melhoria da acuidade visual do olho pregui\u00e7oso.<\/p>\n<p>H\u00e1 tamb\u00e9m formas de penaliza\u00e7\u00e3o parcial como as gotas de atropina no olho forte (que desfocam temporariamente a vis\u00e3o desse olho) em casos selecionados, como alternativa ou complemento ao penso.<\/p>\n<p>&nbsp;<\/p>\n<h4><strong><i>Vantagens e limita\u00e7\u00f5es da oclus\u00e3o<\/i><\/strong><\/h4>\n<p>O penso ocular tem a vantagem de ser utilizado h\u00e1 d\u00e9cadas com efic\u00e1cia comprovada e de baixo custo. No entanto, apresenta v\u00e1rias dificuldades. Muitas crian\u00e7as resistem a usar o penso pelo tempo prescrito \u2013 a ades\u00e3o (compliance) \u00e9 uma quest\u00e3o importante.<\/p>\n<p>O tamp\u00e3o pode causar algum desconforto f\u00edsico e emocional nas crian\u00e7as e nos pais: as crian\u00e7as pequenas podem ficar frustradas por ter a vis\u00e3o tapada e podem sentir-se diferentes ou envergonhadas; a colabora\u00e7\u00e3o \u00e9 habitualmente melhor na escola ao contr\u00e1rio do que os pais pensam\u2026 a ajuda do grupo e a confian\u00e7a nos educadores, nas crian\u00e7as pequenas \u00e9 fundamental.<\/p>\n<p>No entanto, mesmo quando a oclus\u00e3o melhora a vis\u00e3o do olho pregui\u00e7oso, h\u00e1 limita\u00e7\u00f5es importantes: pode ocorrer regress\u00e3o da vis\u00e3o ap\u00f3s terminar o tratamento (em alguns casos espec\u00edficos, e a crian\u00e7a pode continuar a n\u00e3o ter vis\u00e3o binocular normal (estereopsia). Ou seja, o penso trata o olho em separado, mas n\u00e3o ensina os olhos a trabalharem juntos \u2013 muitas crian\u00e7as ambliopes continuam com supress\u00e3o do olho fraco e sem perce\u00e7\u00e3o de profundidade mesmo ap\u00f3s oclus\u00e3o bem-sucedida.<\/p>\n<p>Devido a estes problemas (psicossociais e funcionais), o tratamento com penso pode falhar em uma percentagem significativa de casos por falta de uso consistente.<\/p>\n<p>&nbsp;<\/p>\n<h3><b>Novas Terapias Visuais Digitais (Estimula\u00e7\u00e3o Binocular)<\/b><\/h3>\n<p>Dada a natureza intrinsecamente binocular da ambliopia \u2013 um problema de competi\u00e7\u00e3o entre os dois olhos no c\u00e9rebro \u2013 investigam-se h\u00e1 v\u00e1rios anos terapias alternativas que estimulem ambos os olhos em conjunto, em vez de tapar um olho. Essas abordagens utilizam a chamada estimula\u00e7\u00e3o dicot\u00f3pica, apresentando imagens ou est\u00edmulos diferentes a cada olho de forma controlada. Em termos simples, reduzem-se ou modificam-se os est\u00edmulos do olho forte (por exemplo, baixando o contraste ou desfocando partes da imagem) enquanto se apresenta ao olho fraco uma imagem clara, obrigando o c\u00e9rebro a combinar as duas imagens para completar a cena.<\/p>\n<p>Assim, o olho ambl\u00edope \u00e9 estimulado sem desligar completamente o outro olho, promovendo a coopera\u00e7\u00e3o binocular. Nos \u00faltimos anos, surgiram terapias digitais baseadas nesta ideia \u2013 incluindo dispositivos e softwares interativos \u2013 algumas j\u00e1 aprovadas por entidades reguladoras como a <a href=\"https:\/\/www.fda.gov\/\" target=\"_blank\" rel=\"noopener\">FDA<\/a> nos EUA. A seguir, descrevemos duas das principais op\u00e7\u00f5es dispon\u00edveis atualmente.<\/p>\n<p>&nbsp;<\/p>\n<h3><b>CureSight: Terapia Binocular com Eye-Tracking e V\u00eddeos. <\/b><a href=\"https:\/\/nova-sight.com\/curesight-amblyopia-treatment\/\" target=\"_blank\" rel=\"noopener\"><b>Ler mais<\/b><\/a><\/h3>\n<p>O CureSight \u00e9 um sistema inovador de tratamento da ambliopia realizado em casa, recentemente aprovado pela FDA e marcado CE (Conformidade Europeia). Esta terapia utiliza um tablet especial com rastreamento ocular (eye-tracking) e algoritmos de processamento de imagem em tempo real. A crian\u00e7a usa uns \u00f3culos pr\u00f3prios e assiste a v\u00eddeos ou desenhos animados do seu agrado no ecr\u00e3; simultaneamente, o dispositivo identifica para onde a crian\u00e7a olha e aplica uma desfocagem seletiva na imagem vista pelo olho forte, mantendo a imagem n\u00edtida no olho ambl\u00edope.<\/p>\n<p>Dessa forma, os detalhes centrais s\u00f3 podem ser percebidos pelo olho \u201cpregui\u00e7oso\u201d, encorajando o c\u00e9rebro a usar esse olho sem deixar de ter vis\u00e3o binocular. Em termos pr\u00e1ticos, a crian\u00e7a faz cerca de 1 hora di\u00e1ria de terapia passiva (ver v\u00eddeos) com o aparelho em casa, sob supervis\u00e3o dos pais, tipicamente por 3-5 meses.<\/p>\n<p>&nbsp;<\/p>\n<h4><strong>Efic\u00e1cia e ades\u00e3o<\/strong><\/h4>\n<p>Estudos cl\u00ednicos de grande escala demonstraram que o CureSight tem resultados semelhantes ou superiores ao penso ocular em crian\u00e7as dos 4 aos 8 anos. Num ensaio multic\u00eantrico randomizado com ~150 crian\u00e7as, ap\u00f3s 16 semanas de tratamento, a melhoria da acuidade visual no olho ambl\u00edope foi estatisticamente equivalente entre o grupo CureSight e o grupo da oclus\u00e3o (prova de n\u00e3o-inferioridade face ao tratamento padr\u00e3o).<\/p>\n<p>Al\u00e9m disso, verificou-se uma melhoria maior da vis\u00e3o nas crian\u00e7as que usaram o CureSight de forma consistente comparado \u00e0s que fizeram oclus\u00e3o. Importante salientar, o CureSight mostrou tamb\u00e9m benef\u00edcios binoculares: as crian\u00e7as tratadas com esta terapia melhoraram a estereovis\u00e3o (perce\u00e7\u00e3o de profundidade) e mantiveram esses ganhos ap\u00f3s um ano, algo nem sempre alcan\u00e7ado apenas com o penso.<\/p>\n<p>Outro ponto forte \u00e9 a ades\u00e3o significativamente superior \u2013 no estudo citado, ~94% do tempo de uso prescrito foi cumprido no grupo CureSight vs ~84% no grupo do penso. Isto indica que, em geral, as crian\u00e7as e pais aderem melhor a uma terapia que envolve ver conte\u00fados digitais agrad\u00e1veis do que ao uso prolongado de um tamp\u00e3o. Adicionalmente, os n\u00edveis de satisfa\u00e7\u00e3o parental com o tratamento digital foram elevados, e n\u00e3o se observaram efeitos adversos graves, atestando a seguran\u00e7a da tecnologia.<\/p>\n<p>O CureSight tem vindo a ganhar espa\u00e7o na pr\u00e1tica cl\u00ednica internacional e j\u00e1 est\u00e1 dispon\u00edvel em Portugal, em centros especializados, como op\u00e7\u00e3o complementar no tratamento da ambliopia (sob orienta\u00e7\u00e3o do oftalmologista, que determina os casos indicados).<\/p>\n<p>&nbsp;<\/p>\n<h3><b>AmblyoPlay<span style=\"color: #000000; font-family: Avenir Next;\">: Jogos Interativos e Exerc\u00edcios Visuais em Casa (<\/span><a href=\"https:\/\/www.amblioplay.com\" target=\"_blank\" rel=\"noopener\">Ver website<\/a>)<\/b><\/h3>\n<p>Outra alternativa digital \u00e9 a plataforma AmblyoPlay, que aposta numa terapia visual gameficada atrav\u00e9s de jogos e exerc\u00edcios interativos. Diferente do CureSight (que \u00e9 um dispositivo espec\u00edfico), o AmblyoPlay funciona em computadores ou tablets convencionais, acompanhado de uns \u00f3culos com filtros vermelho\/verde. A crian\u00e7a realiza jogos terap\u00eauticos projetados para treinar habilidades visuais e estimular o olho fraco, tipicamente por 30 minutos di\u00e1rios (por exemplo, duas sess\u00f5es de 15 minutos).<\/p>\n<p>O software adapta automaticamente a dificuldade dos exerc\u00edcios ao desempenho do utilizador, personalizando o treino \u00e0s necessidades de cada um. Esta personaliza\u00e7\u00e3o \u00e9 poss\u00edvel gra\u00e7as a um algoritmo de aprendizagem autom\u00e1tica que analisa os resultados e ajusta o n\u00edvel dos desafios, enquanto os \u00f3culos dic\u00f3pticos dividem as imagens entre os olhos (por exemplo, partes do jogo s\u00e3o vis\u00edveis apenas para o olho esquerdo e outras apenas para o direito).<\/p>\n<p>Assim, tal como no CureSight, garante-se que o olho dominante n\u00e3o veja toda a informa\u00e7\u00e3o sozinho, for\u00e7ando o uso do olho ambl\u00edope com ambos os olhos abertos.<\/p>\n<p>&nbsp;<\/p>\n<h4><strong>Facilidade de uso e motiva\u00e7\u00e3o<\/strong><\/h4>\n<p>Uma vantagem clara do AmblyoPlay \u00e9 a acessibilidade \u2013 por ser um software, pode ser usado em casa com equipamentos comuns, bastando adquirir a aplica\u00e7\u00e3o e os \u00f3culos adequados. Isto torna-o relativamente f\u00e1cil de implementar, inclusive em regi\u00f5es sem acesso a aparelhos mais complexos.<\/p>\n<p>A terapia apresenta-se de forma l\u00fadica, com v\u00e1rios jogos e um sistema de recompensas, o que ajuda a manter a motiva\u00e7\u00e3o da crian\u00e7a durante o tratamento. No entanto, \u00e9 importante real\u00e7ar que o AmblyoPlay geralmente \u00e9 utilizado como complemento ao tratamento principal e n\u00e3o necessariamente como substituto integral da oclus\u00e3o.<\/p>\n<p>Por exemplo, nos per\u00edodos em que a crian\u00e7a n\u00e3o est\u00e1 a usar o penso, ou ap\u00f3s uma fase inicial de oclus\u00e3o, para continuar a melhorar a fun\u00e7\u00e3o binocular de forma agrad\u00e1vel.<\/p>\n<p>&nbsp;<\/p>\n<h3><b>Compara\u00e7\u00e3o das Alternativas Digitais com o Penso Ocular Tradicional<\/b><\/h3>\n<p>Ao comparar as novas terapias digitais com o m\u00e9todo tradicional de oclus\u00e3o, verificam-se vantagens e desvantagens em cada abordagem. O penso ocular continua a ser um tratamento essencial, especialmente em casos de ambliopia mais severa ou em crian\u00e7as muito pequenas.<\/p>\n<p>A oclus\u00e3o tem a vantagem de ser simples (n\u00e3o requer tecnologia complexa) e de efic\u00e1cia comprovada em melhorar a acuidade visual do olho afetado. Contudo, como visto, apresenta desafios de ades\u00e3o e impacto psicossocial que podem comprometer o sucesso do tratamento.<\/p>\n<p>Al\u00e9m disso, por ser um m\u00e9todo monocular, n\u00e3o treina a coopera\u00e7\u00e3o entre os olhos \u2013 muitas crian\u00e7as tratadas apenas com o penso n\u00e3o desenvolvem plenamente a vis\u00e3o tridimensional (estereopsia).<\/p>\n<p>As terapias digitais binoculares surgem justamente para colmatar essas lacunas. O grande benef\u00edcio \u00e9 o treino da vis\u00e3o binocular: ao estimular os dois olhos ao mesmo tempo, estas abordagens podem melhorar a coordena\u00e7\u00e3o binocular e a perce\u00e7\u00e3o de profundidade, algo dif\u00edcil de conseguir apenas tapando o olho bom.<\/p>\n<p>Em termos de resultados, a evid\u00eancia dispon\u00edvel sugere que, em casos leves a moderados, as terapias digitais conseguem melhorias de acuidade visual compar\u00e1veis \u00e0s do penso, com a vantagem adicional de aprimorar a fun\u00e7\u00e3o binocular. Por exemplo, a terapia binocular pode reduzir a supress\u00e3o neural do olho ambl\u00edope de forma mais permanente, enquanto o penso, se usado isoladamente, por vezes leva a reca\u00eddas quando retirado.<\/p>\n<p>Por outro lado, h\u00e1 desafios e limita\u00e7\u00f5es a considerar. Estas tecnologias n\u00e3o substituem totalmente o penso em todos os casos. Em ambliopias causadas por estrabismos de grande \u00e2ngulo, onde a fus\u00e3o binocular \u00e9 praticamente inexistente, pode ser necess\u00e1rio primeiro alinhar\/operar o estrabismo ou usar a oclus\u00e3o tradicional para melhorar a vis\u00e3o antes que uma terapia binocular seja efetiva. Da mesma forma, em crian\u00e7as muito pequenas (beb\u00e9s ou <3 anos), \u00e9 dif\u00edcil aplicar terapias digitais \u2013 nesses casos, tapar o olho ou usar atropina continua a ser a abordagem mais vi\u00e1vel.\n\nAl\u00e9m disso, as terapias digitais requerem recursos tecnol\u00f3gicos (um dispositivo, acesso a software especializado, eventualmente custos de subscri\u00e7\u00e3o) e acompanhamento t\u00e9cnico. Nem todas as fam\u00edlias t\u00eam disponibilidade ou podem arcar com esses meios, e algumas crian\u00e7as com d\u00e9fice de aten\u00e7\u00e3o podem ter dificuldade em seguir mesmo os jogos terap\u00eauticos sem distra\u00e7\u00f5es. Tamb\u00e9m \u00e9 crucial o envolvimento de um profissional para configurar e monitorizar o progresso nesses programas \u2013 n\u00e3o \u00e9 aconselh\u00e1vel iniciar terapias digitais por conta pr\u00f3pria sem orienta\u00e7\u00e3o m\u00e9dica.\n\nEm suma, enquanto as novas terapias oferecem um caminho promissor \u2013 mais amig\u00e1vel e hol\u00edstico \u2013 para tratar a ambliopia, o plano ideal muitas vezes combina o melhor dos dois mundos. Por exemplo, pode-se reduzir o n\u00famero de horas de penso ocular e complementar com exerc\u00edcios binoculares, aumentando a efic\u00e1cia global. Cada crian\u00e7a \u00e9 \u00fanica, e o oftalmologista pedi\u00e1trico avaliar\u00e1 a gravidade da ambliopia e as caracter\u00edsticas do caso para delinear a estrat\u00e9gia adequada, podendo incluir \u00f3culos, penso, atropina e\/ou terapia digital conforme necess\u00e1rio.\n\n&nbsp;\n\n\n<h3><b>Recomenda\u00e7\u00f5es Pr\u00e1ticas para Pais e Cuidadores<\/b><\/h3>\n<p>Para os pais e cuidadores de uma crian\u00e7a com (ou em risco de) <a href=\"https:\/\/www.paulofreitascosta.pt\/pt\/ambliopia\/\">ambliopia<\/a> \u2013 tratamento e alternativas ao penso ocular \u2013 ficam algumas recomenda\u00e7\u00f5es importantes.<\/p>\n<p>&nbsp;<\/p>\n<h4><strong>Realize rastreios visuais precoces<\/strong><\/h4>\n<p>N\u00e3o espere que a crian\u00e7a se queixe de vis\u00e3o \u2013 muitas n\u00e3o se apercebem do problema num olho. Leve-a a consultas de vis\u00e3o de rotina em idade pr\u00e9-escolar (idealmente por volta dos 2-3 anos). O diagn\u00f3stico precoce d\u00e1 \u00e0 crian\u00e7a a melhor chance de recuperar a vis\u00e3o normal.<\/p>\n<p>&nbsp;<\/p>\n<h4><strong>Siga o plano terap\u00eautico individualizado<\/strong><\/h4>\n<p>Se o especialista prescreveu \u00f3culos, use-os em perman\u00eancia conforme indicado. Se recomendou oclus\u00e3o, cumpra rigorosamente as horas di\u00e1rias de penso ou instila\u00e7\u00e3o de atropina. Hoje existem op\u00e7\u00f5es para al\u00e9m do penso tradicional \u2013 terapias binocular digitais, exerc\u00edcios ort\u00f3pticos, etc. \u2013 mas a adequa\u00e7\u00e3o delas depende do caso.
